ROTATOR Staffing Services, Inc. is proud to introduce its optional Voluntary Long Term Disability program. It is an employee-paid program from United States Life. This program will enable you to purchase low-cost disability protection (see rates and provisions) that will provide you disability benefits to the age of 65. Benefits are available in $100 increments with a minimum benefit of $500. You can select any number of units, up to the plan maximum of 60% of your annual salary to a maximum benefit of $6000. MONTHLY BENEFIT PRE-EXISTING CONDITIONS LIMITATION A pre-existing condition is an injury or sickness for which you incurred charges; received medical treatment; consulted a physician; or took prescription drugs for within 3 months before becoming insured under the plan. If a total disability is due to a pre-existing condition, and it begins within 12 months of the date you became insured by the plan, no benefits will be paid. To obtain
